Support for the Money Follows the Person Demonstration

Through a subcontract with New Editions Consulting, TAC provided technical assistance to states participating in the Centers for Medicare and Medicaid Services (CMS) Money Follows the Person (MFP) rebalancing demonstration. Our TA supported program implementation, with an emphasis on building housing and service partnerships and collaborations at the state and local levels to increase access to affordable housing for people with disabilities leaving nursing homes and institutions and to prevent homelessness after discharge from institutional settings. Under the Housing Capacity Building Initiative for Community Living Project, a collaboration of HUD and CMS to support MFP states, TAC provided direct TA, offered virtual peer learning opportunities, and wrote a series of briefs on leveraging Medicaid and using Olmstead principles to develop and implement community-based programs and services for MFP participants.
