Coordinated Entry Improvement in Sonoma County, CA

Under a contract with California’s Department of Housing and Community Development launched in 2019, TAC provided 24 mostly rural and suburban communities with capacity-building assistance to enhance their homeless crisis response systems. Through this contract, TAC supported the Sonoma County Development Commission in an evaluation of the county’s coordinated entry (CE) service delivery system and its countywide street/encampment outreach initiative. The TA focused on addressing key challenges, such as providing mobile CE through street outreach, ensuring reasonable accommodations are provided, establishing adequate geographic coverage, integrating different funding streams, strengthening client access to county services, and evaluating the sustainability of the CE system. TAC delivered a comprehensive white paper with findings and recommendations to drive the county’s CE ongoing improvement efforts, including by incorporating continuous improvement into day-to-day operations.
